1. How is information about my child stored?
For active files: ECSS stores written documentation in secure files at our office at 375 Jefferson under your child’s name. There are copies of reports held at your child’s school in the student support file, which is also in a secure location.
For inactive files: Files created prior to September 2004 that are no longer active are held at the Child Guidance Clinic, 700 Elgin; a copy of this file may also be held at our office on 375 Jefferson. Files created as of September 2004 are stored at 375 Jefferson.
2. Who has access to the information stored about my child?
For active files: Legal guardians have the right to access information about their child at any time. They can give permission for specific individuals or agencies to access information about their child either verbally or by signing a consent form directly. School staff for whom the information is relevant for educational purposes also have access to the information collected and stored by ECSS.
For inactive files: Legal guardians have the right to access information about their child at any time. They must give their consent for others to access stored information about their child after the file is closed.
3. Is there a cost for services?
There is no cost for services provided by the Seven Oaks Educational & Clinical Support Services clinicians.
4. How long will it take for my child to be seen?
It varies. Please contact the clinician directly at your child’s school or at the Seven Oaks Educational & Clinical Support Services office – 582-3383.
